Based in Elizabeth East and Port Adelaide our Transitional Care Team, provide an extensive range of restorative care services to older clients living at home, across the Adelaide metropolitan area.
The Team is seeking a Registered Nurse to support them in achieving a high quality, flexible and responsive service. The primary responsibility of the role is the provision of quality nursing care to program clients through direct nursing tasks, client coordination and case management. The Registered Nurse will assist the team's Clinical Coordinator to provide expert nursing support, advice, leadership and role modelling to other Transitional Care Program staff, consistent with Anglicare's values, policies and procedures.
The Team's approach is person centred and goals based service delivery. The key focus is to ensure the best possible restorative outcomes and long term supports for clients are achieved. The team is multidisciplinary, wholly dedicated to continuous quality improvement and a best practice service model of transitional care for program clients. The role is integral to the provision of skilled nursing services to our team.
The person we seek is courteous, professional in manner and presentation, a true people person, keen to learn; a team player with the capacity to multitask; who knows when to use their initiative and takes direction well. Highly developed communication skills, both written and verbal are crucial to the role. High calibre time management skills are essential to the success of the position, as managing competing demands and ensuring a responsive service are key functions.
This role is ideal for a Registered Nurse who enjoys variety in the work place and who is a confident clinical practitioner. The ideal applicant will demonstrate the communication skills necessary to provide support and advice to program customers and staff working in a multidisciplinary team.

The hours of work are 7.6 hrs (each day) Monday to Friday. Start time vary from 7:30am -9:00am. The Program Office is based at Port Adelaide.
